Supported Living is a service for individuals living in homes that are privately owned, rented, or trust-provided in Pennsylvania. Its goal is to help individuals live independently while staying safe, healthy, and engaged in their communities.
Purpose and Scope
- Provides both direct and indirect support to assist with daily life.
- Focuses on improving independence, productivity, and community participation.
- Support is tailored to each individual’s needs through assessments and person–centered
planning.
Supported Living Services help individuals:
- Perform daily living tasks (hygiene, cooking, cleaning).
- Maintain health and wellness (exercise, safety, emergency response).
- Manage personal medical care (appointments, medications).
- Support mental health and follow behavior plans using trauma-informed care.
- Participate in developing person-centered service plans.
- Handle finances and resources, including ABLE accounts.
- Manage household responsibilities (utilities, safety, maintenance).
- Build communication skills and social connections, inclusive of diverse needs.
- Use transportation options (public transit, driving, carpooling); providers assist with access.
- Maintain healthy roommate relationships and shared household responsibilities.
- Engage in community involvement and civic activities.
- Explore hobbies and personal interests.
Service Delivery
- Billed as daily units and may be provided to needs groups 1 – 4
- Includes times without direct supervision
- Providers must ensure:
- 24/7 on-call support
- Adequate direct care when needed
- Risk management and incident reporting
- Includes oversight by a Supported Living specialist
Restrictions
- Cannot be provided in residential habilitation programs, life-sharing homes, or facility-based settings.
- Must be delivered in a private home or community setting.
