Supported Living is a service for individuals living in homes that are privately owned, rented, or trust-provided in Pennsylvania. Its goal is to help individuals live independently while staying safe, healthy, and engaged in their communities.

Purpose and Scope

  • Provides both direct and indirect support to assist with daily life.
  • Focuses on improving independence, productivity, and community participation.
  • Support is tailored to each individual’s needs through assessments and personcentered
    planning.

Supported Living Services help individuals:

  • Perform daily living tasks (hygiene, cooking, cleaning).
  • Maintain health and wellness (exercise, safety, emergency response).
  • Manage personal medical care (appointments, medications).
  • Support mental health and follow behavior plans using trauma-informed care.
  • Participate in developing person-centered service plans.
  • Handle finances and resources, including ABLE accounts.
  • Manage household responsibilities (utilities, safety, maintenance).
  • Build communication skills and social connections, inclusive of diverse needs.
  • Use transportation options (public transit, driving, carpooling); providers assist with access.
  • Maintain healthy roommate relationships and shared household responsibilities.
  • Engage in community involvement and civic activities.
  • Explore hobbies and personal interests.

Service Delivery

  • Billed as daily units and may be provided to needs groups 1 – 4
  • Includes times without direct supervision
  • Providers must ensure:
    • 24/7 on-call support
    • Adequate direct care when needed
    • Risk management and incident reporting
  • Includes oversight by a Supported Living specialist

Restrictions

  • Cannot be provided in residential habilitation programs, life-sharing homes, or facility-based settings.
  • Must be delivered in a private home or community setting.
